Required elements:

An active WIB Business Account.
Chamber of Commerce Excerpt for the Business (No older than 6 months old).
Copy of ID, Passport, Driver's License for Managing Directors / Authorized Signatories of the Business Account.
Lease / Rental Agreement (for leased/rental properties).
Articles of Incorporation
